Chist hidatic hepatic fistulizat în căile biliare intrahepatice
Hepatic Hydatid Cyst Fistulised in the Intrahepatic Biliary Ducts
(N.F., f., 22 ani / years old)
 
Clinical Manifestations: superior abdominal pains, fever, shivers, nausea, jaundice.
Clinic: dureri în abdomenul superior, febră, frisoane, greață; icter în antecedentele apropiate.
Abdominal Echography: complex structure 83mm diameter with many cystic images placed in the right hepatic lobe.
Echografie abdominală: în lobul drept hepatic formațiune cu diametrul de 83 mm cu numeroase imagini chistice în interior.
Computerized Tomography: voluminous cystic structure, 
9/6 cm, with expended calcifications in the wall, with "hydatid daughter cyst", placed on the 6th - 7th segments -  right hepatic lobe.
Tomografie computerizată: voluminoasă formațiune chistică cu diametrul de 9/6 cm, cu calcificări importante la nivelul peretelui, cu "vezicule fiice" în interior, situată în segmentele VI-VII ale lobului drept hepatic.
Intraoperative Aspect: after dissection of colecyst, it observe enlarged common bile duct (about 3cm).
Aspect intraoperator: după degajarea colecistului se vede CBP enorm dilatată (cca. 3 cm).
 Intraoperative Cholangiography: enlarged common bile duct, with many lacuna images. The left hepatic duct become opaque - the right one doesn't.
Colangiografie operatorie prin puncție: CBP enorm dilatată, conținând numeroase imagini lacunare. Substanța opacă pătrunde în canalul hepatic stâng. Canalul hepatic drept neopacifiat.
Intraoperative Aspect: evidation of many "hydatid daughter cyst" and proligere membrane after transversal choledocothmoy.
Aspect intraoperator: după coledocotomie transversală se extrag numeroase "vezicule fiice" și fragmente de membrană proligeră.
Intraoperative aspect: evidation of hydatid material from common bile duct. 
Aspect intraoperator: golirea CBP de materialul hidatic.
 Intraoperative Cholangiography (after hydatid material evidation): the opaque substance pass through the billiary ducts in the biloculated cystic cavity.
Colangiochistografie operatorie prin sonda Foley: substanța opacă pătrunde din caile biliare în cavitatea chistică biloculată.
Intraoperative Aspect: choledocojejunostomy side-to-side with excluded loop (Roux). The residual cavity is drenaged transfistulous and transcholedocian with tube exteriorisated a la Witzel through anastomosis.
Aspect intraoperator: coledocojejuno anastomoză laterolaterală pe ansă exclusă.Cavitatea reziduală este drenată transfistulos și transcoledocian cu tub exteriorizat a la Witzel prin ansa anastomotică.
 Postoperative Control (cholangiofistulocystography): the opaque substance pass in the residual cavity, in the common bile duct (normal diameter in the inferior half) and anastomotic loop.
Colangiofistulochistografie postoperatorie de control: substanța opacă pătrunde în cavitatea reziduală, 
CBP (care este de calibru normal în jumatatea inferioară) și în ansa anastomotică.
